Welcome to the Builders Exchange Service Association Insurance Program.
We believe the Program to be one of the most competitively priced,
comprehensive and flexible offerings in Employee Benefits anywhere. The
intent of this introduction is to assist you in finding the information
you are interested in. Below is a summary of the available coverages.
All of the plans are stand alone plans. You may sign up for any of the
coverages without getting any other coverage (i.e., dental plan without
health coverage, etc.). You must be a member in good standing to
participate.
Click on the underlined links in the Table of Contents for the benefits
of interest to you. In the case of health coverage, verify your area
(areas are listed by your zip code and/or county). Then, look up the
appropriate premiums per employee by age and dependent status. Should

New SNBE members must elect to enroll in the health plan of their
choice within 90 days of membership approval, or wait until the next open
enrollment period. A member firm in receipt of a renewal letter from a
competitive carrier my join within 30 days of the firms normal
renewal.
Health Coverage: There is a PPO or a POS (See glossary of terms)
and several HMO's to choose from. An employer can choose one plan from
each of the carriers for all their employees or, allow the employees to
choose one plan from each of the health carriers offered (i.e., some
employees in the PPO, others in the HMO of their choice). This is called
Mix & Match coverage. Only one plan per carrier can be selected for
Mix & Match*. Health Net requires a 40% split between H/N &
Kaiser plans. Not all carriers offer providers
(doctors, hospital, etc.) in all areas. Be sure to verify availability of
providers for the plan(s) you are interested in. Also, be sure to compare
plan benefits, and co-payments before making your decision. They will have
a direct bearing on the premiums you pay.
Dental Coverage: There are two plans available. Both plans
allows the insured to go to the dentist of their choice, or use a dentist
in their PDP (Preferred Dentist Program) Network and reduce their
out-of-pocket cost. 100% of eligible employees must enroll for dental
coverage.
Optical Coverage: Blue View one plan both eye wear
and eye care.
Group Life Coverage: There are four options offered. No medical
underwriting is required. If chosen, 100% of eligible employees must
enroll.
Voluntary Term Life: Term Life Insurance is offered on an
individual basis. Age banded rates for Smoker and Non-Smoker
insurers. Offered down to one employee. Available to spouses whether or
not the employee is covered. Medical statements required.
All group plans are Guaranteed Issue (with the exception of
Voluntary Term Life).
